Boscobel School District has teamed up with Emplify Health by Gundersen Boscobel (formerly Gundersen Boscobel Area Hospital and Clinics) to offer at-school sports physicals.

Together, we’re making it easier than ever to ensure your athlete is ready to hit the field, court or track!
Walk-ins only!

* Cash or check only. No insurance filed.
Athletes under age 18 must be accompanied by a parent or have signed parental consent. Not intended to replace your annual wellness exam.
